Current Indications to Surgical Repair of the Aneurysms of Ascending Aorta

چکیده

Normal ascending aorta functions as an elastic reservoir to enhance arterial flow. It stores energy during systole and dissipates it during diastole. The medial layer of the aorta consisting of elastin, collagen, and smooth muscle cells ensures the elasticity of the aortic wall. The ascending segment of the aorta is more elastic than the descending segment, due to its greater concentration of elastic fibres. Aortic aneurysm is defined as a permanent dilation of the aortic wall that exceeds 1.5 times the normal diameter of the aorta in a specified its segment (i.e., the segment of the ascending aorta has a normal value of 24-36 mm, and its dilation for a diameter >40 mm is called aneurysm).

Single-stage, off-pump hybrid repair of extensive aneurysms of the aortic arch and the descending thoracic aorta.

S urgical treatment of aortic arch aneurysms has been challenging. These aneurysms are usually treated with open surgical repair that requires cardiopulmonary bypass (CPB) and hypothermic circulatory arrest (HCA). Furthermore, in cases of aneurysms involving both the aortic arch and the descending thoracic aorta (DTA) extensive surgical incisions or staged procedures are often necessary, increa...

Current Indications for Surgical Repair in Patients with Bicuspid Aortic Valve and Ascending Aortic Ectasia

Preventive surgical repair of the moderately dilated ascending aorta/aortic root in patients with bicuspid aortic valve (BAV) is controversial. Most international reference centers are currently proposing a proactive approach for BAV patients with a maximum ascending aortic/root diameter of 45 mm since the risk of dissection/rupture raises significantly with an aneurysm diameter >50 mm. Current...

Aneurysm of the main stem of the left coronary artery associated with aortic insufficiency and aneurysm of the ascending aorta. Report of a case with successful surgical repair.

A case of aneurysm of the main stem of the left coronary artery associated with aortic insufficiency and an aneurysm of the ascending aorta is reported. The importance of coronary angiography in diagnosing this condition is illustrated. Surgical repair included isolation of the coronary aneurysm and replacement of the ascending aorta and aortic valve, combined with triple aortocoronary saphenou...

Endovascular repair of traumatic pseudoaneurysm of the descending thoracic aorta: report of three cases and review of

Abstract Traumatic descending thoracic aorta pseudo aneurysms have been treated traditionally with open surgery in the past, which have had noticeable rates of mortality and morbidity. A safer method of treatment for this disease is made possible with recent progress in endovascular treatment techniques. In this article, we present three cases of Traumatic descending thoracic aorta pseudo aneu...

The efficacy and short-term results of hybrid thoracic endovascular repair into the ascending aorta for aortic arch pathologies.

OBJECTIVES Conventional total aortic arch repair is a high-risk procedure, particularly for high-risk patients. Although endovascular treatment of aortic arch aneurysm is a recently induced procedure, only a few cases are indicated and outcomes are questionable. Here, we report on the early and short-term results of our surgical procedure, i.e. hybrid arch repair with supra-aortic debranching a...
